My Medical Records (Patient Ally)

Patient Ally is available to established patients of participating Primary Care or Specialty Care providers that are connected with Patient Ally and choose to utilize the Patient Ally feature. Click the following link to go to Patient Ally: My Medical Records

If you have not already set up a Patient Ally account you will need to contact our office so that our staff can initiate the account set up link. If you have forgotten your username or password or are having difficulty click here or call 1-888-PHR-4ALL (747-4255). The hours for Patient Ally technical support are Monday through Friday, 8 am -7 pm CST.

To access Patient Ally’s Patient Portal, you must first receive an email from your provider’s office to initiate the set up process. You must then verify some patient questions and validate your identity using the cell phone number or email on file with the provider. Once you have created a username and a password, you have completed the enrollment process and now may begin using Patient Ally.
